Our app gathers necessary information about a patient to determine hospice appropriateness. The user selects answers to questions about the patient’s ability to perform activities of daily living (ADLs) and about their specific disease (if disease is known).
As you move through the screens you will see information tallied. A score of 50 usually indicates a potential need for hospice. The Palliative Performance Scale increases as indicators reflecting the need for hospice is selected. 50% on this scale also indicates hospice should be considered.
